U.S. School and Government Agency Purchase Orders

Schools and agencies may use Purchase Orders and make payments by check for Miss Jaime, O.T., P.C. products. To initiate the transaction, please follow the two-step process below.

1. Complete the Purchase Order Form, and

2. Download the W-9 for your school and/or purchase tax records.
